HPV v\u00fccudun her yerinde olu\u015fabilir.<\/p>\n<h2>HPV HER ZAMAN KANSERE NEDEN OLUR MU?<\/h2>\n<p>Si\u011filler, mukoza ve deride g\u00f6r\u00fclen iyi huylu lezyonlard\u0131r. Si\u011fillere insan papilloma vir\u00fcs\u00fc (HPV) neden olur ve g\u00fcn\u00fcm\u00fczde 100&#8217;den fazla HPV t\u00fcr\u00fc tan\u0131mlanm\u0131\u015ft\u0131r. HPV enfeksiyonunun birincil belirtileri yayg\u0131n si\u011filler, genital si\u011filler, d\u00fcz si\u011filler, derin palmoplantar si\u011filler (Myrmecia), fokal epitelyal hiperplazi, Epidermodysplasia verruciformis ve Plantar kistleri i\u00e7erir. Si\u011filler do\u011frudan veya dolayl\u0131 temas yoluyla bula\u015fabilir. Normal epitel bariyerini bozan durumlar si\u011fil geli\u015fme olas\u0131l\u0131\u011f\u0131n\u0131 artt\u0131r\u0131r ve tedaviyi zorla\u015ft\u0131rabilir. Si\u011filler genellikle birka\u00e7 y\u0131l i\u00e7inde kendili\u011finden kaybolur.<\/p>\n<p>Baz\u0131 y\u00fcksek riskli HPV alt tipleri, tip 6, 11, 16, 18, 31 ve 35 dahil olmak \u00fczere malignitelerle ili\u015fkilidir. Malign transformasyon genellikle genital si\u011filleri olan hastalarda ve ba\u011f\u0131\u015f\u0131kl\u0131\u011f\u0131 bask\u0131lanm\u0131\u015f hastalarda g\u00f6r\u00fcl\u00fcr. HPV tipleri 5, 8, 20 ve 47, epidermodisplazia verruciformis&#8217;e yol a\u00e7an onkojenik potansiyele sahiptir. Alt tip 6 ve 11 d\u00fc\u015f\u00fck risklidir ve genellikle kondiloma olu\u015fumu ve d\u00fc\u015f\u00fck dereceli kanser \u00f6ncesi lezyonlar ile kendini g\u00f6sterir. HPV alt tipleri 16 ve 18 y\u00fcksek risklidir ve malignitelere ilerleyen y\u00fcksek dereceli intraepitelyal lezyonlardan sorumludur.<\/p>\n<p>HPV&#8217;nin tek ba\u015f\u0131na kansere neden olmad\u0131\u011f\u0131n\u0131, ancak sigara i\u00e7me, folat eksikli\u011fi, UV \u0131\u015f\u0131\u011f\u0131na maruz kalma, ba\u011f\u0131\u015f\u0131kl\u0131\u011f\u0131n bask\u0131lanmas\u0131 ve hamilelik gibi tetikleyiciler ile kanser riskinin artt\u0131\u011f\u0131n\u0131n fark\u0131nda olmak \u00f6nemlidir.<\/p>\n<h2>GEN\u0130TAL S\u0130\u011e\u0130L NED\u0130R?<\/h2>\n<p>Genital si\u011filler (kondiloma ak\u00fcminatum), baz\u0131 insan papilloma vir\u00fcs\u00fc (HPV) t\u00fcrlerinin neden oldu\u011fu cinsel yolla bula\u015fan bir enfeksiyonun klinik belirtileridir. Si\u011filler, genital HPV enfeksiyonlar\u0131n\u0131n tan\u0131nm\u0131\u015f bir semptomudur. HPV&#8217;ye yakalananlar\u0131n yakla\u015f\u0131k %90&#8217;\u0131nda genital si\u011fil geli\u015fmez. Enfekte olanlar\u0131n sadece yakla\u015f\u0131k %10&#8217;u vir\u00fcs\u00fc bula\u015ft\u0131racakt\u0131r. Ayn\u0131 anda farkl\u0131 HPV tipleri ile enfekte olmak m\u00fcmk\u00fcnd\u00fcr.<\/p>\n<h2>HASTALI\u011eIN \u00d6NLENMES\u0130, KONTROL\u00dc VE TEDAV\u0130S\u0130NDE HPV A\u015eISININ ROL\u00dc NED\u0130R?<\/h2>\n<p>Rahim a\u011fz\u0131 kanseri d\u00fcnya \u00e7ap\u0131nda en \u00f6nemli jinekolojik kanserler aras\u0131nda yer almaya devam ediyor. G\u00fcncel verilere g\u00f6re d\u00fcnya \u00e7ap\u0131nda t\u00fcm kanserler aras\u0131nda en yayg\u0131n on d\u00f6rd\u00fcnc\u00fc, kad\u0131nlar aras\u0131nda ise d\u00f6rd\u00fcnc\u00fc s\u0131rada yer al\u0131yor.<\/p>\n<p>Rahim a\u011fz\u0131 kanseri m\u00fcdahalesi birincil ve ikincil korumaya odaklan\u0131r. Az geli\u015fmi\u015f ve geli\u015fmekte olan \u00fclkelerde birincil ve ikincil koruma programlar\u0131 uygulanmazsa, \u00f6n\u00fcm\u00fczdeki 50 y\u0131l i\u00e7inde 44 milyondan fazla kad\u0131na rahim a\u011fz\u0131 kanseri te\u015fhisi konacak. Y\u00fcksek kapsaml\u0131 a\u015f\u0131lama h\u0131zl\u0131 bir \u015fekilde uygulanabilirse, hastal\u0131k y\u00fck\u00fc \u00fczerinde \u00f6nemli bir etki otuz ile k\u0131rk y\u0131l sonra g\u00f6r\u00fclecektir, ancak daha yak\u0131n vadeli etki, HPV a\u015f\u0131lamas\u0131ndan fayda g\u00f6rmeyecek daha ileri ya\u015ftaki kad\u0131nlara servikal tarama yap\u0131lmas\u0131n\u0131 gerektirecektir.<\/p>\n<p>2020&#8217;den itibaren hem HPV a\u015f\u0131lamas\u0131n\u0131n hem de servikal taraman\u0131n geni\u015f kapsam\u0131, 2069 y\u0131l\u0131na kadar 12.5-13.4 milyona kadar serviks kanseri vakas\u0131n\u0131 \u00f6nleme potansiyeline sahiptir.
